The questions in this section are designed to assess the health of your energy-balancing hormones, particularly your sensitivity to insulin. It’s vitally important to gauge your sensitivity to insulin because once you’ve lost normal insulin sensitivity, a small amount of sugar or carbohydrate can lead to very large spikes in your insulin level. Anyone with insulin resistance must strictly limit or entirely avoid the refined starches and sugars that deepen their metabolic damage. The lower your score, the more likely your arterial health is in need of the improvements that following the plan with bring.

Afternoon fatigue is common due to the circadian cycle of cortisol, one of the hormones that the body designed to oppose the effects of insulin and to make us feel energized. Cortisol peaks early in the morning and drops by mid-to late afternoon, so it’s normal to have a small dip in energy at that time of day. When your energy dip is disabling enough that you can’t concentrate, however, that’s an indication that your cortisol system is failing, likely due to being overpowered by chronically high insulin levels.

This frustrating phenomenon is not your imagination. It’s a reflection of the fact that your hormones have spent a lot of time being tooled for the purpose of growth of body fat, and this hormonal signature has dominated your metabolism for long enough that it becomes the default state. It’s a hormonal trap, and you’ve literally eaten yourself into it. The clean-burning fats and slow-digesting carbs you’ll learn to use when you start the plan will help you to eat your way out.

Your fasting blood sugar may be the single most important blood test you can get. If your fasting blood sugar is over 100, you’ve been a sugar burner for long enough to alter your body’s glucose set point (see hypoglycemia discussion in Chapter 3). This is an indication that your body is resistant to insulin and possibly other hormones as well.
If you don’t know your fasting blood glucose, you can call your doctor. Doctors almost always order fasting blood tests as part of a physical, and will almost always order glucose as one of the tests. If your blood sugar is just a little high, they don’t, however, always tell you, so it’s worth calling your doctor’s office to find out the number yourself.
If you haven’t had any blood tests done in the past year or so, I recommend paying a visit to get the tests performed.
